Application Instructions
To be considered for a Scholarship, all applicants must:

Write a 300 - 500 word letter describing your future academic goals and highlighting why you are applying for this Scholarship. This letter is a very important part of your application. |

Be a Canadian citizen or landed immigrant. |
 |
Be over 17 years of age. |

Have been either treated for some form of childhood cancer or still be on treatment. |
|
Provide a copy of the letter of acceptance to a university, college or any post secondary school educational program (or Pharmacy School or Medical School if applying for the Teva Canada Scholarship) and a copy of your last year's transcript if you are not a first year student. (The acceptance letter may be forwarded at a later date.) |

Provide a statement from your doctor / pediatrician / oncologist stating that you have had some form of childhood cancer. |

Print and complete the application form and submit all documents before deadline of June 30. |
Procedure:
- The Scholarships are awarded in September/October.
- You will be contacted if application is not complete.
- You will be notified about the decision.
- Scholarship recipients are listed on our website and may be contacted by people/companies who have "adopted" (funded) their scholarship.
- Submit the completed application by June 30 via mail or fax, to:
